Library users in the Richardson Library or the Smith Music Library may NOT be asked to answer a survey or questionnaire without prior approval from the Dean of Libraries. The Dean may request the HSU Research Review Committee to approve the project. Therefore, adequate lead-time will need to be provided before the questionnaires or surveys are utilized.
